Strategies to Reduce Readmissions

A list of Strategies to Reduce Readmissions was developed by the Readmissions Quality Collaborative Steering Committee, based on a review of the literature.  Each participating clinical service selects and implements one or more of the strategies listed.  In addition, each inpatient or emergency service implements at least one enhanced discharge planning process.

Clinical Forms Developed for the Collaborative

The Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) team is developing tools to support project-related clinical processes:

  • Readmission Risk Assessment Form Editable Word Version This form may be used unchanged, or it may be tailored to each hospital’s procedures or selected strategies.  For example, hospitals may add eligibility criteria for a specific intervention.
  • Other forms as needed
