Learning Collaborative Calls

Monthly Learning Collaborative Calls

Monthly learning collaborative calls are attended by members of the Quality Improvement (QI) teams from participating hospitals. These calls, in which participants share effective strategies and lessons learned, are a key component of the learning collaborative model. A rotation schedule ensures that each team has the opportunity to participate actively by reporting on its QI project throughout the year.

Strategies to Reduce Readmissions

Each call in the ‚ÄúStrategies to Reduce Readmissions‚ÄĚ focuses on one of the core strategies (129kb) recommended by the Collaborative. These calls provide an opportunity for collaborative participants implementing the strategy in question to share ideas with each other and/or to hear from outside experts.
