UPDATE – 09/24/21

Please be advised that on September 24th, the Excel Spreadsheets for the 36 Slot NYC and 36 Slot Rest of State/Downstate budgets have been updated/replaced. There were additional formula errors that were not caught in the fix/update for 9/21/21.

If you are using/have used either of these budget templates, please discontinue and use the budget templates with the date 9/24/21 next to them.

We apologize for the inconvenience and confusion.

09/21/21

Please be advised that the four Appendix B budget templates (36 & 48 Slot Upstate and 36 & 48 Slot Downstate) that were placed in the Pre-Submission Upload page in the Grants Gateway had formatting and calculation errors. Please disregard these templates and use (only) the ones provided here on OMH’s website where the RFP is located.

In addition, there are now two new Appendix B budget templates (36 Slot Rest of State/Downstate and 48 Slot Rest of State/Downstate) that are to be used by the following counties who are on an Upstate contract cycle, but receive a Downstate rate due to their geographic location: Orange, Putnam, Rockland, Westchester, Nassau and Suffolk. The remaining Upstate counties will use the 36 & 48 Slot Rest of State/Upstate templates.

Due to these changes for the Appendix B budget templates, the new due date of the proposals is Friday, October 1, 2021 at 4:00 pm.
