Their representative told me that they do not have a standard form.
When the community pharmacy processes the claim, it will reject if it needs a prior approval. If the pharmacist calls the plan, the plan will automatically generate a patient specific prior approval form and the plan will fax it to the prescriber.
The representative said that there would be a fax number preprinted on the form so that the prescriber would know where to send the completed form.
