Adult ACT RFP - 14 New Teams

The New York State (NYS) Office of Mental Health (OMH) announces the availability of funds for the development of 14 Assertive Community Treatment (ACT) teams:

    •  ( 1 ) 68 slot team covering Suffolk County
    •  ( 1 ) 48 slot team covering Livingston, Genesee, and Northern Steuben counties
    •  ( 1 ) 48 slot team covering Rensselaer and lower Saratoga counties
    •  ( 1 ) 48 slot team covering Jefferson and Lewis counties
    •  ( 2 ) 68 slot team in Brooklyn
    •  ( 3 ) 68 slot team in Manhattan
    •  ( 3 ) 68 slot teams in The Bronx
    •  ( 2 ) 68 slot teams in The Queens

The ACT teams will serve individuals who are diagnosed with Serious Mental Illness (SMI) and have not been successfully engaged by the traditional mental health treatment and rehabilitation system.

These individuals may be high users of emergency and/or crisis services, have co-occurring substance use disorders, are isolated from community supports (including family), are in danger of losing their housing/becoming homeless, are homeless, and/or have histories of involvement with the criminal justice system.
