It has come our attention that there is confusion regarding proposal templates mentioned in the RFA instructions.

To clarify, there is no proposal template required for this RFA. You must fill out the Attestation for Providers form provided in the attachments/comments section of SFS along with the other required documentation. After filling it out you will upload the attestation to the question asking:

“Question 1 - Please upload your PDF response. Please combine everything (including any attachments referenced in the Proposal Narrative) into one PDF. The template is located in the attachments section of this Bid Event in SFS. There are no character limits or word counts applicable in this document. Please note that the Budget Narrative is included in the template document and should be answered there and not in a separate upload. PLEASE NOTE THAT UPLOADS CAN NOT EXCEED 20MB.”
