MH263040 - Safe Option Support (SOS) Program: Critical Time Intervention (CTI) Teams

OMH is soliciting applications to support 7 Safe Options Support Critical Time Intervention Teams in New York City.

One team will be awarded in each of the following areas:

• Upper Manhattan
• Lower Manhattan
• Manhattan/86 Street to 14 Street 5th Avenue East to the River
• Manhattan/125 Street to 50th Street, 5 Avenue/Central Park West to Hudson River
• Middle Manhattan/Penn Station
• Upper East Side /East Harlem/Grace Church School
• 40th Street to 53rd Street from 6th to 8th Avenues, plus Restaurant Row

SOS CTI Teams use an evidence-based approach to provide intensive outreach, engagement, and care coordination services. This funding will support an estimated ~4,500 street homeless individuals.
