Assertive Community Treatment (ACT) Program

Overview

Assertive Community Treatment takes a "whole team" approach to treating serious mental illness. The ACT Team promotes recovery by providing services within our community. The goal of ACT is to help people become independent and integrate into the community as they experience recovery. The team offers support to help people find housing and employment, nurture relationships, and get relief from symptoms and medication side effects.

The ACT program is unique in that all services can be delivered within the community, either at home or another preferred location. The team provides 24-hour emergency assistance.

Who Will Benefit

The ACT program is open to people between 18 and 64 who have a mental health diagnosis. It serves individuals who haven't had success with traditional treatment providers (clinics, day treatment programs).
