AIMS:
Support clinics in assessing and improving client engagement, with a focus on high-risk clients. Project elements include:
- Map, analyze and enhance client experience and clinic workflow
- Monitor client engagement with feedback to individual clinicians
- Screening, tracking and outreach protocols for high-risk clients
- Screen Social Determinants of Health & address actionable social needs
