The Engaging Clinic Quality Improvement Collaborative

AIMS:
Support clinics in assessing and improving client engagement, with a focus on high-risk clients. Project elements include:

  1. Map, analyze and enhance client experience and clinic workflow
  2. Monitor client engagement with feedback to individual clinicians
  3. Screening, tracking and outreach protocols for high-risk clients
  4. Screen Social Determinants of Health & address actionable social needs
