PSYCKES Project Indicators
The two project options for the CQI Initiative are Health Promotion and Coordination and Behavioral Health Care Coordination. Indicator sets (117kb) aligned with the two projects are available in the Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES).
- The Health Promotion and Coordination indicators focus on ensuring that appropriate planning and coordination takes place for individuals at risk for high utilization of medical inpatient services and medical emergency room (ER) services. Indicators also focus on individuals who may be in need of appropriate laboratory monitoring/screening and outpatient medical visits. The indicator set includes a summary indicator identifying the number of unique individuals who meet criteria for any of the Health Promotion and Coordination indicators. All measures apply to both adults and children except for the preventable hospitalization indicators which are for adults only. The set includes seven indicators. - The Behavioral Health (BH) Care Coordination indicators focus on enhancing planning and coordination for individuals with histories of high utilization of inpatient and emergency room behavioral health services, as well as individuals with a diagnosis of schizophrenia, bipolar, or depression and who might have concerns related to adherence and discontinuation of certain medications. The indicator set includes a summary indicator identifying the number of unique individuals who meet criteria for any of the Behavioral Health Care Coordination indicators.
