Intervention Checklists

Quality improvement (QI) is a data-driven process.  Knowing what interventions have been delivered to which clients is critical in assessing the success of the project and whether changes are needed.  In order to support this important work, OMH has developed two checklists that clinics can use to track the delivery of interventions:  an Individual Intervention Checklist and a Group Intervention Checklist.  Each checklist is available in a version tailored to either the Health Promotion and Coordination project or the Behavioral Health Care Coordination project.

These checklists are optional.  If your clinic has developed a form for collecting data and it works well for your clinic, continue to use your tracking mechanism. 

  1. The Individual Intervention Checklist can be used by clinicians to track interventions delivered to a specific client. 
  2. The Group Intervention Checklist can be used to track interventions delivered to a group of clients.

See the directions for more information about the checklists and an illustration of a completed form. 

Each form is customizable.  Clinics should ensure that forms reflect their clients’ interventions and can be integrated into workflow processes for collecting and tallying project data. 

These forms are for your internal use; clinics should not submit them to OMH.
