
Monthly Data Reporting
Clinics participating in the OMH CQI Initiative for Health Promotion and Care Coordination are required to report monthly on project activities for the previous calendar month. Monthly reporting is via an on-line survey. A unique link is sent to clinics each month and is also posted on the website in the News section.
Each month OMH provides clinics with a report that aggregates and summarizes the survey data.
- Milestone Reporting (through September 2014)
Clinics reported on the achievement of project milestones. On the survey, each clinic used its Project Planning Form (PPF) to help answer a set of nine questions for project activities tailored for each project: - Quantitative Reporting (beginning in October 2014 for September 2014 project activities)
Each month clinics will be asked to report on:- newly identified clients
- total number of active identified clients
- interventions delivered to clients
- use of the PSYCKES application to review clinical summaries
The survey includes guidelines for answering the quantitative questions.
