CQI Handbook

The CQI Handbook (1.1Mb) for the Continuous Quality Improvement Initiative for Health Promotion and Care Coordination provides an overview of the initiative, project requirements and quality concerns.  It also covers the mechanics of implementing a CQI project using the Six Sigma Define-Measure-Analyze-Improve-Control (DMAIC) quality improvement model.
